Cougars News · IMPORTANT: Additional IHSAA Consent/Release form required in order to participate in athletics during the 20-21 school year

The IHSAA has added an additional document that must be completed BEFORE a student-athlete will be allowed to participate once workouts begin.


The certificate is available to print out below:

*PLEASE NOTE, this form requires parent/guardian signatures and must be in hand (along with having a valid IHSAA Physical form on file  (*19-20 forms dated after 4/1/18 will be accepted per guidance statement on 4/22 below) in order to participate in practices/workouts.


Hard copies are also available in the pamphlet box on the left side of the main NPHS entrance (Door S1).



Again, these forms will need to be turned in to coaches prior to participating in workouts which are scheduled to begin July 6th.  Additional information will be provided by specific coaches and/or the NPHS Athletic Department as we get closer to the 7/6/2020 start date.



A REMINDER:  That due to the COVID-19 pandemic during the 2019-20 school year and the continued stress on the medical community, the Indiana High School Athletic Association, Inc. will waive the provisions of Rule 3-10 for the 2020-21 school year.  Effective immediately, valid physical and consent forms presented to member schools on behalf of eligible students during the 2019-20 school year shall be valid during the upcoming 2020-21 school year.

These considerations have been vetted through the Committee on Sports Medicine of the Indiana State Medical Association and the Indiana High School Athletic Association, Inc.  After careful consideration, these accommodations have been developed.
